Tria Health offers a program for those with diabetes and/or hypertension, which includes supplies and visits with a pharmacist.
Eligible participants have access to speak with a pharmacist and coach to monitor their high blood pressure, manage diabetes, and get medication management support.
Members have access to a personal health coach, and free blood pressure and diabetes supplies. Members can also earn up to $150 in prepaid Mastercards within a 12-month period.
