To learn more about the CDPHP Care Team, call (888) 942-3747, Monday through Friday from 8:30 a.m. to 5:00 p.m. ET. This dedicated phone line is answered by trained intake specialists who will talk with you to understand your needs and connect you with the most appropriate member of our Care Team.
A CDPHP care manager can help you navigate serious chronic conditions over a lengthy timespan or can offer short-term support with things like appointment reminders, medication education, and healthy lifestyle planning. A care manager can send you health information by secured email or standard mail and offer referrals to community agencies for supports not covered through your CDPHP health plan.
Your privacy is important to CDPHP, so a care manager will not share your health information with anyone else unless you tell CDPHP it is okay. Care managers have consent forms that you can sign to give CDPHP permission to speak with friends, family, providers, or other supports if you wish. Utilizing the supports of the CDPHP Care Team is completely voluntary.
