Even with the best medical care and relationship with your health care provider, serious and ongoing health problems can be overwhelming. As a Blue Cross and Blue Shield of Illinois (BCBSIL) member, you have the chance to work with a care manager who can provide support during your maternity care.
Your care manager can be a major source of support through the many months of pregnancy.
What is a care manager?
Care managers are registered nurses who can help with everything from helping you understanding a diagnosis given by your health care provider to educating you on lifestyle options and self-care skills for managing that condition. Care managers can help with education about a condition, preparation for doctor’s appointments, and helping you stick to the treatment plan established by your health care provider.
Care managers can also offer support for these conditions and more:
- Gestational Diabetes
- Gestational Hypertension
- Multiple Pregnancy
- Placenta Previa
- Preeclampsia
- Short Cervix
Typically, care managers reach out to members directly, by telephone or letter, to offer their help. This often happens after a doctor’s appointment or hospital stay. But if you feel like you could benefit from the support of a care manager now, please call (888) 421-7781.
Please note that Care Managers cannot provide clinical services such as medical advice, medical or medication recommendations, diagnoses, treatments, or prescriptions. Care Managers provide education and resources to help support you through your health care journey but all conclusions and decisions regarding your care are made by you and your health care provider.
