As an Anthem PPO Diamond Value member, you and your dependents will receive benefits for different provider visits and therapies for those with autism spectrum disorders.
For in-network office visits you’ll pay just a $40 copay, and 30% of costs (after meeting your deductible) for all other in-network outpatient treatments. For out-of-network office visits you’ll pay 50% of costs after meeting your deductible.
For autism therapy and services, you’ll also pay the full cost of each visit until meeting your deductible. After you’ve met your deductible, you’ll pay 30% of the cost for in-network therapy, and 50% of the cost when you go out-of-network.
